Predictive Patterns of Glutamine Synthetase Immunohistochemical Staining in CTNNB1-mutated Hepatocellular Adenomas

چکیده

Some hepatocellular adenoma (HCA) subtypes are characterized by different CTNNB1 mutations, leading to beta-catenin activation levels, hence variable immunostaining patterns of glutamine synthetase (GS) expression, and risks malignant transformation. In a retrospective multicentric study 63 resected inflammatory (n=33) noninflammatory (n=30) molecularly confirmed CTNNB1-mutated b-(I)HCA, we investigated the predictive potential 3 known GS as markers for exon 3, 7/8 mutations. Pattern 1 (diffuse homogenous) allowed recognition 17/21 non-S45 mutated b-(I)HCA. 2 heterogenous) identified all b-(I)HCA harboring S45 mutation (20/20). (focal patchy) distinguished 12/22 with both b-HCA b-IHCA showed GS+/CD34? rim diffuse CD34 positivity in center lesion. Interobserver reproducibility was excellent Comparative analysis molecular data 83% 80% sensitivity (b-HCA/b-IHCA) 100% specificity non-S45. For S45, 93% 92% (b-HCA/b-IHCA). 7/8, 55% 96% Preliminary from 16 preoperative needle biopsies same patients suggest that this panel may also be applicable small samples. surgically HCA, distinct can reliably predict which relevant because higher risk The third pattern, although specific, less sensitive identification mutation, but is valuable aid indicate either an or mutation.

Hepatocyte Nuclear Factor 1α-Mutated Hepatocellular Adenomas: An Atypical Presentation

Hepatocellular adenomas (HCAs) are rare benign monoclonal hepatic tumors that commonly occur in females (3-4 per 100,000 women) due to the use of oral contraceptives, its primary risk factor. Recently, HCAs have been classified into 4 distinct subtypes according to genotypic and phenotypic characteristics and clinical features: inflammatory HCA (40-50%), which are hypervascular with marked peli...

Histological and Immunohistochemical Revision of Hepatocellular Adenomas: A Learning Experience

Light has been shed on the genotype/phenotype correlation in hepatocellular adenoma (HCA) recognizing HNF1 α -inactivated HCA (H-HCA), inflammatory HCA (IHCA), and β -catenin-activated HCA (b-HCA). We reviewed retrospectively our surgical HCA series to learn how to recognize the different subtypes histopathologically and how to interpret adequately their immunohistochemical staining. From Janua...

Regulation of synthesis of glutamine synthetase by adenylylated glutamine synthetase.

We have examined three mutants of Klebsiella aerogenes whose genetic lesions (glnB, glnD, and glnE) are in loci unlinked to the structural gene for glutamine sythetase (glnA) and in which the control of both the level and state of adenylylation of glutamine synthetase is altered. Each mutation alters a different component of the adenylylation system of glutamine synthetase [L-glutamate:ammonia ...

Activating mutations in CTNNB1 in aldosterone producing adenomas.

Primary aldosteronism (PA) is the most common cause of secondary hypertension with a prevalence of 5-10% in unreferred hypertensive patients. Aldosterone producing adenomas (APAs) constitute a large proportion of PA cases and represent a surgically correctable form of the disease. The WNT signaling pathway is activated in APAs. In other tumors, a frequent cause of aberrant WNT signaling is muta...
